Covid-19 Release
As the health and safety of the clubbers, volunteers, staff, and families is our priority, we are focused on taking all reasonable measures to prevent the spread of COVID-19 at our club. Ultimately, the choice for your child to participate in Natomas Awana Club (“AWANA”) is a personal one and you are in control. If at any time you are uncomfortable with the risks of COVID-19 in an in-person club setting, please do not send your child to club.
By signing this form, you indicate that you are aware that participation in AWANA at our host facility, The Crossing Church, may cause injury or illness such as, but not limited to COVID-19. For each time you choose to bring your child to AWANA, you attest that they have not experienced symptoms of fever, fatigue, difficulty in breathing, or dry cough or exhibited any other symptoms relating to COVID- 19 within the last 14 days. You attest that your child has not, nor have any member(s) of their household, been diagnosed to be infected with COVID-19 within the last 14 days, nor have they had any known exposure to COVID-19 within the last 14 days.
With full knowledge of the risks involved, I hereby release, waive, discharge Natomas Awana Club, Natomas Homeschool Alliance, The Crossing Church, Awana Clubs International, all volunteers, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19.
I agree to indemnify, defend, and hold harmless the Natomas Awana Club, Natomas Homeschool Alliance, The Crossing Church, Awana Clubs International, and their representatives from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to COVID-19.
